Nelson was one of four players, along with Kyle Schwarber, Rhys Hoskins and Garrett Stubbs, to be on the active roster for all 162 games during the 2022 regular season. He was omitted from the postseason rosters during the Wild Card Series and the NLCS, but added during the NLDS and the World Series. He pitched a scoreless eighth inning against the Astros in World Series Game 3, in which the Phillies won, 7-0.

Nelson spent all of 2023, save the final day of the regular season, in the minor leagues mostly with the Lehigh Valley IronPigs. He also made a few rehab appearances with Clearwater and Reading. In 25 minor league starts, Nelson was 7-3 with a 3.83 ERA. He was activated for Game 162 against the Mets, pitching extremely well for 5 1/3 innings and earning the win.
